A packet of six forms completed by each state department and submitted to the Executive Budget Office, Department of Finance. The packet is evaluated by the Executive Budget Office and used to draft the Governor's budget recommendation. The following forms compose the packet: Estimated Condition of Funds (EBO-1); Summary Budget Request (EBO-2); Capital Expenditures (EBO-3); Performance Statement (EBO-4); Summary of Program or Program Elements (EBO-5); Personnel Classification (EBO-6).
